Required Health Forms & Medication Updates for 2026–2027 School Year

As previously mentioned, the state requires that schools collect physical exam reports for students in grades 7, and 10 each year. If you have not already submitted your child's physical exam report already, it needs to be submitted ASAP. The report must include the students' updated immunizations. Religious Exemptions need be submitted yearly for immunization compliance. Religious Exemptions must come from the parent/guardian and state, in writing, that a vaccine conflicts with sincerely held religious beliefs. 

Please keep in mind that all school orders must be updated yearly at the start of the new school year. If your child will be taking medications during school hours and/or has a special healthcare need necessitating Doctor’s Orders or an Individualized Healthcare Plan, please contact your child’s physician, or PCP, to have them generate new orders for the upcoming school year. Orders need to be renewed for scheduled, as needed, AND emergency medications, including inhalers and automatic epinephrine injectors. If your child wishes to self-carry an inhaler or epinephrine, please ask your doctor to make a note of this on the orders. Without an active annual order, no medication can be administered.
