📝 Annual Meal Modification Forms

If your student requires any meal modifications due to food allergies, intolerances, medical needs, or other dietary requirements, please remember that we need an updated Meal Modification Form each school year.
The form is attached to this post. Please complete and return it to your school nurse, who will work with the Nutrition Services team to ensure we have the most current information and can properly support your student.
We appreciate our families and staff helping us keep our students safe, healthy, and well-fed.
We are looking forward to seeing all of the kids and serving them delicious, nutritious meals throughout the school year! 🥕🍎🥪
