The following forms must be returned for student registration:

• Student Registration Form
  • Race-Ethnicity Codes
• Sworn Statement of Residency Form
• Residency Affidavit
• Landlord Property Owner Verification
• Home Language Survey
• McKinney-Vento Eligibility Questionnaire
• Custodian Form for Sharing Information
• Responsible Adult Affidavit
• Military Family Status
• Millbury Health Services Packet (during the school year)
• Preliminary Medical Information
• Free and Reduced School Meals Application (if applicable)

In addition to the completed forms, you must provide the following:

• Student’s Birth Certificate
• Current physical and immunization information (if the previous school does not provide)
• Two Proof of Residency   (see Sworn Statement of Residency Form for all required documents)

Finally, your child’s previous school will need to provide the following, which will be requested once the Records Release Form is received signed: 

• Complete transcript
• MA Transfer Card (if transferring from a Massachusetts school)
• Discipline Record
• MCAS scores (if transferring from a Massachusetts school)
