Substitute Teaching

If you are interesting in substitute teaching, you will need to complete the following forms:

CORI Form- All employees must have an active CORI form in order to work for Millbury Public Schools. You must also submit a copy of your driver's license.   CORI_FORM-1.pdf

Fingerprints- All employees must have their fingerprints taken. If you have worked for another school district, you can ask to have a letter of suitability sent on your behalf. If you have never had your fingerprints taken, log on to identogo to make an appointment. The District code is 01860000.   http://www.identogo.com
