Substitute Teaching

If you are interesting in substitute teaching, you will need to complete the following forms:

CORI Form- All employees must have an active CORI form in order to work for Millbury Public Schools. You must also submit a copy of your driver's license.   CORI_FORM-1.pdf

Fingerprints- All employees must have their fingerprints taken. If you have worked for another school district, you can ask to have a letter of suitability sent on your behalf. If you fingerprints were taken more than seven years ago, you will need to have them taken again. If you have never had your fingerprints taken or need to have them taken again, log on to identogo to make an appointment. Onboarding Sheet- The attached information must be submitted in order to create an online account where you can fill out your payroll paperwork. /onboarindgprepsheet
